This appears to be the mid 90s remake of math rabbit, not the original 1986 version.

I still own my copy of this game. Never helped me enjoy math, but the prizes were amusing.

Even though my computer can't process this game, I'm going through a serious nostalgia trip from just looking at the screenshots you've posted. I'm so glad I finally remembered the name of this game.
